Conducts a comprehensive sensitive assessment with the client and family/support network as appropriate, in order to develop a plan of care to meet identified needs. __ Arranges and coordinates service referrals for the client/family as needed; provides ongoing monitoring of the activities implemented to carry out the plan of care. Refers clients/families to public benefits programs as eligible. __ Provides regular contact with enrolled clients and documents all contacts and service coordination activities in client records as required by #program guidelines. __ Educates clients/families about the basic medical and psychosocial aspects of disease in order to optimize adherence to care. __ Provides proactive follow up, tracking symptoms and treatment responses. __ Adheres to all applicable state laws and the department’s rules and regulations regarding programs. __ Stays abreast of new resources, #programs and developments. __ Participates in continuous #training and education, including attendance at staff meeting and community meetings as assigned.
