Conduct Start of Care (SOC) visits Complete OASIS documentation accurately and timely Perform skilled nursing assessments Educate patients and caregivers Collaborate with interdisciplinary teams Ensure compliance with care plans and regulations What we're looking for Active Registered Nurse (RN) license in the state of Michigan OASIS Experience a plus Minimum of one year of nursing experience, preferably in home health care or acute care setting. Strong assessment and documentation skills.
