Responsibilities: Assist in the development, review, and revision of the plan of care. Provide nursing care per the physician’s plan of treatment, individualized plan of care, as authorized by the client’s payer source. Observe, record, and report any changes in the client's condition to appropriate personnel and/or physician. Deliver ongoing instruction and supervision to clients, families, and health team personnel in the plan of treatment. Monitor and document the client's condition and provided care services using electronic charting. Participate in education programs, quality assessment activities, nurse meetings, and other staff meetings as required.
