This position serves as the essential link between the clinic and the community, ensuring that patients have access to the resources, support, and advocacy they need to thrive. If you’re passionate about equity, relationship‑building, and helping families overcome barriers, this role offers the opportunity to create lasting change in the lives of those who need it most. What You’ll Do Build trusting, empowering relationships with patients and families to understand their needs and connect them with community and healthcare resources. Serve as a key liaison between the clinic and community organizations, strengthening the network of support available to families. Document resource needs, referrals, and follow‑up activities in the electronic health record with accuracy and timeliness. Collaborate closely with the multidisciplinary healthcare team to ensure coordinated, patient‑centered care. Participate in program planning, implementation, and evaluation to enhance services and improve outcomes. Advocate for patients by identifying barriers, brainstorming solutions, and persistently pursuing pathways that promote long‑term stability. Engage in ongoing professional development, including completing Sickle Cell Community Health Worker certification within one year of hire. Promote cultural humility, equity, and empowerment in every interaction. What You Bring Associate’s degree in Human Services, Public Health, or a related field. At least one year of experience working directly with patients or families in a healthcare or community setting. Experience supporting underserved communities and navigating social service systems. Commitment to completing Sickle Cell Community Health Worker Training and certification within one year. Ability to build strong, cross‑cultural relationships grounded in empathy and respect. A persistent, solutions‑focused mindset and comfort navigating complex systems. Strong communication skills, both written and verbal. Proficiency with computers, databases, electronic medical records, email, and communication systems. Ability to work independently, manage multiple priorities, and apply new skills with attention to detail. A collaborative spirit and enthusiasm for shared learning and teamwork.
