Prior to submitting your application, please review and update (if necessary) the information in your candidate profile as it will transfer to your application.  Job Title: Physical Therapist, Outpatient Neuro Rehabilitation Department: Ambulatory Services | OP Rehab at Dublin The Neuro Rehab Team Member   is current ly licensed to practice   therapy in the State of Ohio. This individual will work within a   team oriented   multi-disciplinary rehabilitation program .    These services are offered via outpatient service sites to patients who have disorders and conditions associated primarily with neurologic injuries .   The rehabilitation team members report to   the rehabilitation clinic manager   as part of the   Ambulatory   R ehabilitation Services   structure .     The Neuro Rehab Team Member   works under the general supervision of the Director of Rehabilitation Services and under direct   supervision of their clinic manager.   The Rehab Team Member   evaluates and treats patients in accordance with the laws and rules that govern   the practice of their profession   in the state of Ohio.   Although team members rep ort directly to the clinic manager , they are also responsible to each other, to those they supervise   and to other participants included in patient care activities.    Each Rehabilitation Team M ember is first a part of a group effort to provide care within a continuous performance improvement environment, and secondly a distinct professional who provides specific contributions to the team approach .    Team members assess patients and   establish   together plans of care ; provide a wide variety of treatment techniques; provide education and support to families; and   facilitate   continuing care planning .    Team members evaluate quality of services and   participate   in outcomes management activities .       Minimum Qualifications        All Rehab Team Members   must hold current licensure with the State of Ohio Occupational Therapy, Physical Ther apy and Athletic Training Board or the Ohio Board of Speech Language Pathology and Audiology   and meet the educational and/or certification requirements   established   by their professions .     All academic degrees must   have been   granted from an accredited institution .    Current American Heart Association BLS certification preferred .        Maintains professional licensure;   demonstrates   commitment to continuing education; meets mandatory education and health requirements;   demonstrates   competence in technical, interpersonal, and cognitive skills   required   to meet essential job functions .     Maintain current American Heart Association BLS certification.    Knowledge of growth and development and an understanding of the range of treatments necessary to meet the age specific needs o f the patient population served   (Check those that apply):      Job Relationships         Physical Therapist Assistants,   Occupational Therapy Assistants,   Athletic Trainers, rehabilitation aides, student medical assistants, student interns, and volunteers as assigned.    Interacts daily with patients, families, caregivers, faculty members (physician and psychologists), nurses, and their support staff , other team members (inpatient and outpatient), clerical staff, managers and administrators, admissions personnel,   visitors   and vendors .    May interact with referral sources and payors if   requested .     Responsible to:    Rehabilitation Clinic Manager    Physical/Visual/Mental Requirements    The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job .    Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions.    While performing the duties of this job, the employee must be able to:   Communicate, clearly with all members of the health care team,   patients   and families.   Concentrate and   attend to   a task or series of tasks.   Maintain visual attention and concentration, depth   perception   and visual field for patient treatment tasks and safety.   Identify   and respond to emergency signals and indicators and respond quickly.   Frequen tly lift and support weight of 8 0+ lbs. ( i.e.   transferring patients from one surface to another).   Frequently push or pull 80+ lbs.
