With more than 40 years of experience forging multisector partnerships and with expertise in science, economics, technology, advocacy, and dozens of other specialties, PATH develops and scales up innovative solutions to the world’s most pressing heath challenges.  PATH’s work on Structural Heart Disease (SHD) PATH is implementing the "Young Hearts, Strong Futures" pilot project in Sitapur district, Uttar Pradesh, to improve outcomes for Structural Heart Diseases (SHDs). The project focuses on improving children’s health by implementing early interventions which will reduce childhood cardiac morbidities and mortalities. This will be achieved by integrating health system-driven innovative tools into school-based screening program and community interventions. Key components of this initiative include: AI-Enabled Screening: Deploying AI-based devices and standardized questionnaires to enhance the screening capacity of the public health system. Strengthening Referral Pathways: Establishing a robust ecosystem for early diagnosis, referral, and management of SHD cases (Congenital Heart Disease & Rheumatic Heart Disease) from schools and Community Health Centers (CHCs) to District Early Intervention Centers (DEIC) and tertiary care. Capacity Building: Training Rashtriya Bal Swasthya Karyakram (RBSK) teams and frontline workers to screen and refer for SHDs effectively. About the position The Block Monitor (BM) will facilitate the field implementation of the SHD project across assigned blocks in Sitapur district. This role serves as the essential link between government administrative bodies and field health teams, orchestrating adherence to micro-plans and screening drives. The incumbent will provide hands-on field support by overseeing the use of AI-enabled diagnostic tools, leading community sensitization efforts, and managing a seamless patient referral and follow-up network. The role requires meticulous data integrity, involving the tracking of treatment adherence and the reporting of the field activities and data. Responsibilities of Block Monitor:                             Implementation of the SHD project across 10 blocks in Sitapur district. Work closely with block level administration and implementation of the SHD project at block and field levels. Liaison with Block-level health and administrative officials (RBSK teams, MOIC, BEO, CDPO) to facilitate smooth screening activities and referrals. Orchestrating adherence to micro-plans and screening drives and supervising the screening activities across ten assigned blocks to ensure project alignment Providing hands-on field support by overseeing the deployment and usage of AI-enabled diagnostic tools and data entry tools Managing a robust two-way referral system and follow-up network and tracking of treatment adherence Leading community sensitization efforts by planning, supporting and overseeing awareness activities Facilitate training sessions for field teams and government healthcare staff on project protocols and technical tools. Consolidating the project related data, preparing field reports and sharing the block level reports with the District Program Officer.   Undertake other tasks which may be assigned from time to time by the state and district leadership. Required Experience and Qualifications : Essential - Graduate in Science / Public Health/Social Work/Development Studies. Desirable: Post-Graduate in Public Health/Social Work/Development Studies. Experience: • 2-3 years of experience in the healthcare or development sector, specifically in field implementation. • Prior experience working with government health systems, specifically in programs like RBSK, or similar NHM programs, is desirable. Skills/ Abilities Mandatory: Valid two-wheeler driving license and ability to ride a motorcycle/ scooter for extensive field travel. Sound understanding of community mobilization and working with ASHAs, ANMs, and Anganwadi workers. Good verbal and written communication skills in Hindi (essential) and basic English. Basic computer literacy (MS Office, Google Sheets) for reporting and data entry. Travel: Willingness to travel extensively within the 10 assigned blocks to visit CHCs, PHCs, schools, and Anganwadi centers for monitoring and facilitation of project activities. Strong negotiating and communication skills. Ability to manage multiple tasks, set priorities, work independently, and maintain a high level of accountability.
