Your Opportunity   As a Case Manager with the Spin 180 program in Darwin, you will deliver purposeful, person-centred case management to young people ages 12-25 who are experiencing or at risk of homelessness, disengagement from education, employment, or family and community connections. Working directly with clients, you will support positive change by addressing the complex and intersecting challenges impacting stability, wellbeing and independence. Spin 180 takes a holistic, culturally responsive approach, supporting young people to strengthen life skills, rebuild connections and create pathways away from homelessness and anti-social behaviour. Based within the Darwin service, this role involves hands-on engagement with clients, collaboration with families and community partners, and active participation in the day-to-day delivery of intensive, strengths-based support that enables young people to transition toward long-term housing, education, employment and community participation. Your Key Responsibilities   Respond to referrals from internal and external services and undertake comprehensive intake and suitability assessments. Conduct formal, face-to-face client assessments using approved tools, ensuring accurate and timely documentation. Develop and implement holistic, strength-based case plans that reflect clients’ cultural strengths, goals and identified needs. Provide intensive, ongoing case management support, reviewing progress and adjusting plans to support independence and wellbeing. Link clients with appropriate internal and external services, including housing, AOD, FDV, health, education and employment supports. Maintain professional boundaries while building client capacity, resilience and self-management skills. Undertake advocacy, coordination and administrative case management tasks, including referrals, reports, support letters and stakeholder engagement. Requirements for Success   Degree in Community Services with at least one year’s experience, or an Associate Diploma or lesser qualification with substantial relevant experience. Demonstrated experience supporting clients with complex needs, including homelessness, AOD, FDV, mental health and unemployment. Proven case management capability, including assessment, planning, advocacy, referral and accurate case note recording. Strong knowledge of the homelessness and community services sector, with the ability to build effective networks and partnerships. Experience working with diverse client cohorts, particularly young people and families, using culturally responsive and trauma-informed approaches. Excellent verbal and written communication skills, with confidence using client management systems and Microsoft Office applications. Strong organisational skills, including time management, report writing, data collection and the ability to manage competing priorities within a team environment. Full driver licence with no restrictions.
