Nurse Educator(NE) 專案衛教師/個案管理師 Act as a Program Nurse educator for Patient Service 主要工作職責 Major Duties Performed (Main Functions) •協助專案說明與個案收案並依照規範協助相關文件歸檔。 Assist in project explanations and enroll case, ensuring compliance with documentation standards. •協助進行疾病與藥品衛教，回診前評估並回報醫療端。 Assist in health education regarding diseases and medications, conduct pre-visit assessments, and report findings to healthcare professionals. •依規範進行不良事件通報。 Report adverse events according to established guidelines. •個案管理、回診追蹤及資料整理。 Case management, follow up on appointments, and organize data effectively. •依規範繳交專案服務報告。 Submit service reports in compliance with regulations. •協助舉辦病友會 (依各專案需求)。 Assist in organizing patient association as needed. 學歷、經驗及專業知識要求 Education, Experience, & Specialized Knowledge Required •醫藥相關科系，副學士學位或學士學位或同等學歷。 Medical and Health-Related Fields, Associate Degree or Bachelor’s degree or equivalent. •2年以上衛教師/個管師/臨床護理師經驗 2 years of experience as a Nurse Educator/Case Manager/Clinical Nurse. •良好的人際交往/問題解決能力。 Good interpersonal and problem-solving skills. •團隊合作精神，具備良好的溝通技巧，並以客戶服務為導向。 Team player with strong communication skills and a customer service-oriented approach. •基本的電腦文書能力，包括 Word 和 Excel。 Basic PC literacy skills, including Word and Excel. •具備衛教師相關經驗者最佳。 Experience as a nurse educator is preferred.
