426 Michigan Internal Medicine is an innovative, busy rising risk practice looking for a partner passionate about primary care, learning and growing to join their value site on Grand Rapid's Medical Mile. The office currently has three Internal Medicine physicians and advanced practice providers providing a full range of care to their patients.  A team-based model of care is utilized to include a pharmacist, social worker, RN care manager, behavioral health, population health nurse and excellent medical assistants and staff.  Provider and team member collaboration, coordination, and innovation is considered key to the practice's success and the workspace is built to accommodate this.
