Job Profile Summary Under limited supervision, is responsible for the clinical management of assigned neonates in the Newborn Intensive Care Unit and Intermediate Care Nursery, and is responsible, under supervision, for stand-by at risk deliveries and neonatal resuscitation in the Labor and Delivery Room. Education: Certified Nurse Practitioner Program or Physician Assistant   Licensure: • Current registration with the Maryland State Board of Examiners of Nurses as a Neonatal Nurse Practitioner or Physician Assistant • Heart saver (CPR Certification).   Experience: At least two years of experience in a Level III or Level IV NICU   Skills: • A high level of analytical ability is required in order to assess patients and implement appropriate intervention. • Substantial interpersonal and verbal communication skills required to effectively interact with staff, physicians, patients, and family members. Principal Duties and Responsibilities: Operations Management: • With minimal supervision facilitates a multifunctional team to ensure Anatomic Pathology operations provide efficient, high quality, cost-effective laboratory services. Develops and implements work processes for all areas of responsibility. Establishes polices for all clinical and business operational tasks within assigned areas Strategic Operations Management: • Directs the evaluation of new technologies, procedures and supplies. Provides counsel on new test or service implementation. Plans, implements and monitors achievement of strategic business objectives. Communicates initiatives to appropriate personnel. Identifies, implements and monitors new practice opportunities for growth. Provides investigation and resolution for issues brought up by clinicians and customers related to Anatomic Pathology testing Service Excellence: • Monitors, investigates and responds to customer, physician and pathologist feedback, modifying technical and office systems, processes as needed • Maintain or improves Key Performance Indicators Human Resources Functions: • Manages human resources within established productivity guidelines and the personnel budget. Facilitates training and development efforts. Maintains employee competencies. Monitors and reviews work performance of the staff. Interviews and selects personnel. Provides development opportunities to staff • Responsible for the supervision of Histotechnologists, Laboratory Assistants, Pathology Office Manager, Secretaries, Pathologist Assistants and Technical Specialists Quality and Patient Safety Initiatives: • Oversees staff involvement in quality control and improvement efforts, general safety, and infection control procedures • Initiates, monitors, evaluates performance improvement on a unit level as needed • Monitors trends, reviews and revises systems/processes as appropriate Financial Management: • Manages the area’s budget and prepares variance reports as appropriate. Prepares operating budget and makes recommendations for capital budget in consultation with appropriate Hospital staff. Maintains inventory and analyzes cost of supplies and services. Manages accounts payable issues. Processes payroll for assigned areas. Compliance with Regulatory Agencies: • Ensures that all criteria of inspection and accreditation agencies are met and that proficiency survey test results are correct. Ensures standards practices are met and incorporates compliance with regulatory laws. • Provides admission, daily, and discharge physicals on babies admitted to the Neonatal Intensive Care Unit and Intermediate Care Nursery. Handles problems of the infant and does counseling with parents regarding their infant’s care and needs. • Establishes medical diagnosis for common short-term or chronic stable health problems. The following problems may be evaluated independently or in consultation with the attending neonatologist or attending neonatal consulting pediatrician when necessary. This list includes, but is not limited to: 1. Hyperbilirubinemia 2. Feeding problems 3. Methadone withdrawal/neonatal abstinence after stabilization 4. Transient respiratory problems 5. Dislocated and/or subluxated hips 6. Conjunctivitis 7. Dermatological problems 8. GU problems 9. Sepsis 10. Respiratory distress 11. Congenital heart disease 12. Anemia 13. Apnea 14. Hematologic problems 15. Hypoglycemia • Manages problems as assigned by the attending neonatologist or attending neonatal consulting pediatrician. • Orders, performs, and interprets laboratory tests (including diagnostic and invasive procedures). The following laboratory tests can be ordered, obtained, and the results interpreted by the Neonatal Nurse Practitioner. This list is not inclusive: 1. Blood work, including CBC, sedimentation rate, Hgb, blood gases, electrophoresis, blood type and cross and match, Coombs, STS, VDRL, electrolytes, liver enzymes, thyroid function, viral antibody titers, direct and indirect bilirubin estimation. 2. Cultures including urine, blood, nasopharyngeal, throat, wound, rectal, stool, eye, C.S.F., other body fluids 3. Examination of urine, urinalysis, microscopic examination, Gram stain, ferric chloride 4. Examination of stool for occult blood 5. Hematocrits 6. Skin scrapings 7. Blood gases • The following non-invasive procedures can be ordered independently or with physician consultation, including, but not limited to: 1. X-rays 2. EKG 3. Sonograms 4. EEG 5. CT scan 6. MRI • The following invasive procedures can be ordered and performed by the Neonatal Nurse Practitioner. Again, the list is not inclusive. 1. Venipuncture to obtain blood tests as listed above 2. Spinal tap 3. Arterial punctures 4. Arterial catheterization 5. Endotracheal intubation 6. Umbilical arterial catheterization 7. Umbilical venous catheterization 8. Thoracostomy 9. Thoracentesis 10. Urinary catheterization 11. Suprapubic tap 12. Paracentesis 13. Central venous catheterization • Prescribe drugs. • Is authorized to prescribe non-controlled substances independently or in collaboration with a physician. The following drugs may be prescribed in the Neonatal Intensive Care Unit and Intermediate Care Nursery. This list is not inclusive. 1. Erythromycin 1% ophthalmic ointment 2. Vitamin K1 3. Glycerine suppository 4. Antibiotics 5. Mycostatin ointment & suspension 6. Anticonvulsants 7. Sympathomimetics 8. Vaso-active agents 9. Vitamins 10. Electrolytes 11. Diuretics 12. Immunizations 13. Topical preparations 14. Blood & blood products • Orders and/or performs the following therapeutic or corrective measures. They include, but not limited to: 1. Nasopharyngeal suctioning 2. Chest PT 3. Clean and dress abrasions and minor lacerations 4. Ligate extra digits 5. Triple diapering 6. Phototherapy 7. CPR 8. Intubation 9. Mechanical ventilation 10. Thoracentesis 11. Hyperalimentation 12. Vaso-active agents 13. Paracentesis 14. Oxygen therapy • Refers patients to appropriate licensed physician or health care provider. • Provides emergency care, including, but not limited to: 1. Initiating CPR 2. Administering O2 3. Initiating the IV in an emergency 4. Intubate and ventilate 5. Order medications during an emergency • Attends deliveries when requested by obstetricians. This may include appropriate resuscitation and stabilization of the newborn.
