As a Clinical Pharmacist specializing in Cardiology, you will be an integral part of our nationally recognized Heart, Vascular & Thoracic Institute, delivering expert pharmaceutical care to a diverse and complex patient population. This role focuses primarily on heart, vascular and thoracic patients, including those in general cardiology and thoracic care, individuals preparing for or recovering from cardiac and vascular surgery, and heart and lung transplant recipients. In this position, you will coordinate and optimize medication therapy on assigned clinical units, collaborating closely with physicians and interdisciplinary teams through active participation in patient care rounds. While the focus will be 100% Cardiology, there may also be opportunities to cross-train in the main inpatient pharmacy and IV room, broadening your clinical expertise and impact. A caregiver in this role will work part time and predominately daytime hours, with an evening rotation every eight weeks. The schedule includes every 8th weekend and 1 holiday per year. Hours may vary based on assignment, ranging from as early as 6:30am to 3:00pm to as late as 2:00pm to 10:30pm during evening weeks, with the most common shift being 7:00am to 3:30pm. A caregiver who excels in this role will:   Provide safe, efficient, patient-centered, evidence-based medication services.  Verify medication orders and/or dispenses medications accurately according to state and federal regulations.   Document services in a complete, accurate and timely manner.  Actively contribute to medication safety initiatives.  Ensure all pharmacy operations are in compliance with national accreditation standards.  Directs staff appropriately to provide safe and efficient care and maintain professional working relationships with all caregivers.  Educate and train students, residents, other caregivers and/or patients.   Minimum qualifications for the ideal future caregiver include:   PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy  Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire  Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience  Preferred qualifications for the ideal future caregiver include:  Board Certification   Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology) Our caregivers continue to create the best outcomes for our patients across each of our facilities.
