In this role, you will manage clinical and operational tasks, address urgent issues, attend codes and stroke alerts, and support satellite operations with an emphasis on critical care needs. You will be part of Cleveland Clinic’s Inpatient Pharmacy team, which delivers comprehensive distributive and clinical services across 60 nursing units, 80 operating rooms, 4 PACUs, and 10 cath labs, supporting more than 1,440 inpatient beds. If you are not attending but would like additional information or wish to connect with someone from our team, please apply here or email   ginthec@ccf.org .   A caregiver in this role will work Monday - Friday with a rotating weekend expectation and every 8th weekend rotating holiday expectations.   A caregiver who excels in this role will:    Coordinate and provide pharmaceutical care for patients.    Participate in patient care rounds.    Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients.    Provide general and patient-specific drug information.    Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy.      Minimum qualifications for the ideal future caregiver include:    PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy    If hired for a specialty area where specialty residency training is available, then specialty residency is required     Current state licensure as a Registered Pharmacist (RPh) OR eligible for reciprocation in the current state within 90 days of hire    Completion of a post-graduate year 2 residency (PGY2) or fellowship OR completion of a post-graduate year 1 residency (PGY-1) and two years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmaceutical monitoring OR four years of experience as a clinical specialist or an equivalent role providing pharmaceutical care and performing pharmacotherapeutic monitoring    For individuals required to administer immunizations: current Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire    For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or certification obtained within 1 year of hire      Preferred qualifications for the ideal future caregiver include:    Board Certification      Our caregivers continue to create the best outcomes for our patients across each of our facilities.
