As a Home Health Aide on our H ospice team , you will care for patients where they need you most, whether at home, a nursing facility , assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones.     A caregiver in this position works nights, 7:00pm - 11:00pm. This would be 5 days a week being 4 hour shifts.   A caregiver who excels in this role will:    Perform basic patient care activities.    Assist with procedures ordered by physician , with supervis ion from an RN, PT, OT or ST as appropriate .    Assist patients with tending to personal care, activities of daily living and transfers/transport.   Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.   Assist with keeping the patient's environment clean and orderly.   Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.   Escalate change in patient condition to patient’s physician as appropriate .       Minimum qualifications for the ideal future caregiver include:    High S chool D iploma or GED   State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry   One year of clinical experience as an A ide in an acute, rehab or skilled nursing setting   Valid D river’s L icense (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage     Basic Life Support (BLS) Certification through the American Heart Association (AHA) within orientation period     Preferred qualifications for the ideal future caregiver include:   Hospice and Palliative Care Nurse Assistant (CHPNA) C ertification   Home C are experience     Our caregivers continue to create the best outcomes for our patients across each of our facilities. For field staff only: Able to drive to patients’ homes and/or facilities. Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
