As a Social Worker on our team, you will apply your expertise in crisis intervention, resource navigation, mediation, counseling, and patient empowerment to support individuals and families from diverse backgrounds and with varying needs. Your work will directly influence patients' lives, helping them move toward stability, healing, and improved overall well-being.   A caregiver in this position works  M-F: 8:00am—5:00pm.     A caregiver who excels in this role will:    Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emot ional, financial and environmental problems.    Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, e ducation, advocacy, referrals, conflict resolution, crisis intervention, problem solving a nd coordinating family/team meetings ).   Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.    Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.    Maintain a network of resources for consultation and connect patients with community resources.    Supervise S ocial W ork I nterns and assist with training new caregivers.    Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.      Minimum qualifications for the ideal future caregiver include:    Master’s D egree in S ocial W ork from a CSWE accredited program   Current state licensure as a Licensed Social Worker (LSW)     State licensure as a L icensed Independent Social Worker (LISW) within five years of hire   Two years of experience in a hospital or healthcare setting     Our caregivers continue to create the best outcomes for our patients across each of our facilities. May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
