Using your knowledge and education, you will develop personalized treatment plans for pediatric, critically ill and high-risk patients according to their medical histories, individual needs and goals. Working with these populations requires compassion and sensitivity, so maintaining an understanding attitude and supportive presence is essential. Ultimately, your role is integral to our comprehensive model of care, encompassing every aspect of wellness and health.  A caregiver in this position works varying shits from 7:30AM – 5:00PM, with rotating weekend and one yearly holiday requirements.  A caregiver who excels in this role will:   Provide individualized medical nutrition therapy, education and counseling to patients.  Implement nutritional care plans, communicate plans, establish a follow-up system and maintain documentation.  Write changes for diet orders, initiate nutrient intake assessments, assess patients for nutritional deficiencies, evaluate fluids and electrolytes and review laboratory and clinical progress notes.  Inform physicians of patient nutritional status and need for diet changes.   Create and revise education materials, participate in program development and develop, implement and market services.     Support the dietetic internship program as a supervised practice rotation preceptor and class instructor.  Minimum qualifications for the ideal future caregiver include:   Bachelor’s Degree in Nutrition, Dietetics or a related field  Current licensure as a Licensed Dietitian (LD)   Active certification as a Registered Dietitian (RD)  Preferred qualifications for the ideal future caregiver include:  One year of experience as a Registered Dietitian in Clinical Nutrition OR successful completion of CCF Dietetic Internship Program  Pediatric experience  Our caregivers continue to create the best outcomes for our patients across each of our facilities. Must have physical ability to measure and weigh patients.
