Job Description The Behavioral Health Consultant supports Primary Care physicians/Health Home teams in identifying and behaviorally intervening with Health Home enrollees who could benefit from behavioral intervention. Minimum Requirements Master’s Degree Social Work or demonstrated/verified equivalent in combination of education and experience Licensed Social Worker - LMSW Minimum of three (3) years experience working in a healthcare related setting Good written and verbal communication skills Must be able to read and speak the English language Preferred Clinical Licensed Social Worker - LCSW
