What You’ll Do: Care Coordinator Extraordinaire: Design, implement, and evaluate treatment plans by collaborating with your healthcare team, ensuring each patient receives the best care at every stage of their journey. Health Home Champion: Provide and manage care for Health Home enrollees, tracking their healthcare outcomes and making adjustments to treatment plans as needed. Team Collaborator: Actively participate in and coordinate patient care with other team members, fostering a holistic approach to treatment and recovery. Evidence-Based Expert: Use evidence-based information to drive care decisions, ensuring that every patient receives high-quality, up-to-date treatment. What You Need to Bring: Education: BSN (or equivalent combination of ADN and relevant experience). Licensure: Current Missouri RN license. Certification: Current BCLS (Basic Life Support) certification. Experience: A minimum of three years working in a healthcare setting. Communication Skills: Strong written and verbal communication skills—you’ll need to effectively coordinate with patients and the healthcare team. Tech-Savvy: Proficient in Microsoft Word, Excel, and PowerPoint. Language Proficiency: Must be able to read and speak English fluently. Preferred Qualifications: Community Service Experience: Any experience working in community health is a plus. Multi-language Skills: Ability to speak multiple languages is a bonus!
