Job Description Primary Duties & Responsibilities: Arranges and coordinates referrals from testing sites, hospitals, clients living with HIV, and community partners for individuals newly diagnosed with HIV or those out of care. #Conducts initial intakes, eligibility screenings, and needs assessments. Educates clients/families about HIV care, treatment options, and available support services in order to optimize adherence to care. #Assigns clients to case #managers based on needs, eligibility, and program requirements. Schedules appointments with medical providers and case managers as needed. Provides proactive follow-up to confirm clients are successfully linked to medical care and support services. Maintains accurate and timely documentation in electronic databases and client records. Tracks referrals, outcomes, and linkage-to-care metrics for quality assurance and reporting. Communicates effectively with providers, case managers, and testing staff to ensure coordinated client support. Adheres to all state laws and the department’s rules and regulations regarding confidentiality, HIPAA, and institutional standards. Collaborates with case managers from other regions when a client resides outside of program service boundaries. Participates in continuous training and education, including attendance at staff meeting and community meetings as assigned. Functions as part of a multidisciplinary HIV specialty healthcare team and participates in case conferences, team meetings, and professional development trainings as scheduled.
